Philippines: 14-309: Psychiatrist Trainer- Philippines

PROGRAM SUMMARY:
On November 8, 2013 Super-Typhoon Haiyan (Yolanda) made landfall in the Visayas region of the Philippines and left widespread devastation in its wake, damaging infrastructure, disrupting livelihoods, and halting agricultural production. For many of the 14 million people affected by the storm, Typhoon Haiyan caused not only bodily harm, but sparked a major increase in psychological distress. Confronted with losses of loved ones, homes, and sources of income, many survivors presented with signs of mental stress and trauma. In the immediate aftermath of the storm, most patients seen at International Medical Corps mobile medical units (MMUs) in Tacloban (Leyte) and Roxas (Capiz) reported suffering the loss of home or livelihood, and approximately 75% of IMC’s patients reported psychological distress.
The expansion of the current MHPSS program will bring it from nine to 17 municipalities across Leyte Province, covering International Medical Corps’ entire geographic area of responsibility – and indirectly reaching a population of over 760,000 people. International Medical Corps will build on the training implemented during the first phase of the program to improve outreach and community-level support. International Medical Corps will work with traditional community-based support systems, involving social workers, faith leaders and other key persons at the community level who can provide basic psychosocial support, linking them to doctors and nurses trained on the mhGAP curriculum to strengthen and expand the continuum of care from the community to health facilities and back down.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

  1. Train primary health care (PHC) staff to identify and manage those with mental or psychosocial disturbances through regular seminars, on-the-job training and regular supervision of treatment.
  2. Assist the PHC staff in creating a clinical service for the mentally ill that is integrated into their working week, through specially allocated clinical time or an equivalent mechanism.
  3. Respond to psychiatric emergencies in conjunction with PHC staff.
  4. Keep a comprehensive audit of all clinical work conducted and train PHC staff to similarly audit their work in order to provide comprehensive patient contact and management data.
  5. Assume responsibility for monitoring and reporting on the work to demonstrate the extent to which the indicators listed in the proposals are being met.
  6. Liaise effectively with IMC global mental health advisor to provide training materials and tools for review, send regular technical reports (quarterly) and receive input on development of materials.
  7. Coordinate project monitoring and evaluation activities, with attention to record of lessons learned through evidence-based practice for the purpose of project replication or expansion.
  8. Work within the framework of the IASC guidelines for mental health and psychosocial support in emergency settings and Sphere standards
  9. Help in the promotion of IASC guidelines
  10. Represent IMC as requested in various for a including cluster and working group meetings
  11. Work collaboratively with other agencies, local and international and with relevant government bodies
  12. Actively promote PSEA (Prevention of Sexual Exploitation and Abuse) standards within International Medical Corps and amongst beneficiaries served by International Medical Corps.Qualifications:1. Holds a degree in medicine or psychiatric nursing, has completed residency and holds current license/registration to practice medicine or psychiatric nursing.
  13. Has a minimum 3 years clinical experience in a hospital and / or community based mental health, preferably in humanitarian/low resource settings
  14. Training experience required.
  15. Strong organizational and supervisory skills.
  16. Strong interpersonal skills and the ability to work within different cultural environments.
  17. Honest, hard working and a self-motivated person
  18. Ability to work within a team structure or in isolation, flexible, and can cope with stressful workloads and working with limited resources.

SPARK is achieving its mission by organizing business plan competitions, business skills training, SME coaching & mentoring, business incubation, SME financing, intensive higher vocational summer courses, curriculum development and quality assurance at universities and higher vocational education institutions.

Terms of Reference – ToT Business Growth Training, Libya

Background & Purpose

In December 2012, DFID awarded a 14-month grant for the Benghazi Entrepreneurship Centre to support both young aspiring entrepreneurs and existing businesses with a view to creating employment opportunities for youth in Eastern Libya.

Through four business plan competitions (BPCs), the BEC will select promising business concepts and support their creators with Business Skills Training, coaching and mentoring services, with the aim to help them establish or grow their businesses. Finalists in the growth BPC receive targeted training modules that fit their specific needs. Training will be delivered to beneficiaries by both staff of the BEC and local external trainers.

Development of training material targeting existing entrepreneurs shall be based on SPARK’s own approach. The focus of this training is on existing entrepreneurs who want to develop and grow their business and enable them to expand their operations and create jobs for young Libyans. The entrepreneurs that we are targeting with this training are literate, have been operating their own business for a couple of years and are motivated in expanding and growing their activities. They have experience in running businesses. The training is supposed to help them identify their business’ strengths and weaknesses, identify the market’s opportunities for them to grow in and the threats and challenges that they will be facing. In addition, this training aims at giving them a more comprehensive view of what is growth, how it can be accomplished and managed. The trainees are expected to come up, at the end of the training, with a SWOT analysis, and / or a balance scorecard to support them, in a more practical way, develop their business.

In general, the training will consist of the following topics covered in +/- 3 days and including very practical and hands-on exercises: 1. What is growth;2. How to accomplish growth;3. How to manage growth;4. SWOT/Balance scorecard;5. Personalized business expansion roadmap.

At the end of this training period, the trainer should be able to identify the weaknesses of the trainees in the different business management areas (i.e. marketing, human resources, finance, etc...) in order to provide an additional 1 to 2 days in depth specific training sessions.

Scope:

The current assignment is to:• Contribute to the development of SPARK’s Business Growth training materials (power point and trainers manual) including practical group and individual exercises that will help trainees in identifying their strengths and weaknesses and develop a sound expansion plan;• Familiarize the core staff of the BEC (Project Director, Training Coordinator, Project Coordinator and Controller) and local external trainers with the training modules and instruct them to the effective use for ToT of local experts and delivery to project beneficiaries;• To make necessary contextual adaptations to the training material (powerpoints and trainers’ manual); fine tuning of Arabic language versions of training material; development or adaptation of examples, exercises or cases studies fitting with the local context.

The assignment deliverables are to be provided as following:• Contribution to development of Business Growth training materials during a 10 day workday period;• BEC staff and project beneficiaries training session during a ten days workday mission of the contracted consultant to Libya, to take place in October 2013.

Tasks & Responsibilities:

Develop SPARK Business Growth training materials (trainer’s manual, trainees’ handouts and powerpoints) in line with SPARK Quality Assurance procedure in English and Arabic• Develop a trainer’s guideline to identify trainees’ business management weaknesses based on SPARK approach;• Provide targeted and tailored ToT-training to BEC core staff and SPARK local trainers in Benghazi;• Provide the Business Growth training to growth oriented entrepreneurs;• Cover both content and didactical principles;• Adapt training materials to Libyan context and deliver these in ready to use form to BEC staff;• Provide a final assignment report.

Debriefing & Reporting:

Reporting takes place during the mission through daily debriefing by email to SPARK (Karen Blanken and Mazen Al Kassem). Within 10 days of completion of the mission, a concise report, including key observations and recommendations is to be submitted.

Results:

• Business Growth training for existing entrepreneurs developed and fine-tuned in Arabic and English;• Business Growth training materials contextualized for Libya, including relevant business cases and examples;• Business Growth training conducted in Libya to BEC staff, SPARK local experts and business growth oriented entrepreneurs.

Time Frame:

Remuneration:

The contractor does not enter the SPARK employment scheme. The contractor is responsible for all additional expenses related to the remuneration, e.g. taxes, pension fees etc.

Remuneration for the assignment is € 400 euro/day (gross). The assignment covers a ten days curriculum development, ten day mission, three days for preparation and one day for reporting / debriefing (total 24 days).

The successful candidate will primarily be responsible for the development and delivery of the International Management Methodology (IMM) training programme. The IMM promotes a more consistent approach to managing all the work of International, a large component being the Project Cycle Methodology which covers projects and programmes. This was introduced to users in 2011 and continues to be developed and refined following user feedback.

The P&A Trainer will also take over responsibility for the existing training programme for the Programme & Information Management System (PIMS). PIMS was successfully rolled out to the International Division in October 2011 and is a user friendly and intuitive system which holds all of the International Division’s work in a structured and logical way.

The P&A Trainer will be responsible for the development and delivery of training making use of the latest technologies including WebEx and e-learning as appropriate.The post holder will work with a range of internal stakeholders, and be responsible for developing a coherent training programme in liaison with the Learning and Development department. Opportunity Description

We are seeking one or two people interested in joining our team in Madison, Wisconsin as a Partner (minimum 5 - 10 years experience) or Associate (minimum 3 - 5 years experience). BACKGROUND:

Initially based in Amman, Internews will implement a pan-regional Humanitarian Communications Program (HCP) that will provide Syrian IDPs, refugees, host communities and diaspora groups across the region with vital humanitarian and community information that affects their everyday lives.

Through humanitarian radio broadcasts and TV programs, community participation through SMS/mobile technology, social media channels, crisis mapping, a dedicated online platform and other relevant and secure communication channels, the HCP will disseminate timely, actionable and useful humanitarian information for target audiences about e.g. how to access and register for aid, provision of food, water, shelter, security conditions, referral services, health practices and services, to name a few.

Equally important, the HCP will foster audiences’ participation and bring voices and community feedback into humanitarian operations. The aim is to support 2-way communication between target audiences and humanitarian service providers.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following; other duties may be assigned:

Embedded production: Embedded within the newsroom of main local media partner in Amman, responsible for the production of high quality humanitarian radio programming covering key themes including but not limited to services available to affected communities, health, registration, or protection issues.Training/Mentoring: In coordination with the Humanitarian Radio Advisor, design and implement the training and mentoring program for reporters of local radio partner assigned to this program. S/he will assist the Humanitarian Radio Advisor, overall responsible for the radio component of this project, in the design and implementation of training/mentoring programs of local reporters from other countries, as necessary, as required byRelationship with local radio partner: Manage the day-to-day relationship with the main partner local radio station partner, including ensuring that the programming is broadcast.Production of daily rundowns: Compile rundowns of the humanitarian radio programs for distribution to the wider humanitarian community through the Humanitarian Liaison Officer and/other potential ways.Support liaison with humanitarian organizations: Under the supervision of the Humanitarian Radio Advisor (primary contact for this component) and in coordination with the Humanitarian Liaison Officer, support in the liaison with humanitarian agencies for the production of humanitarian news and information to disseminate important messages, news and information for affected communities.Any other duties as required.

This project comes as reinforcement to the 4 year-long Multi-Stakeholder Initiativestarted in January 2010 with the aim of improving access to quality rehabilitation services in the region. Rehabilitation services in Iraqi Kurdistan are particularly affected by a lack of technical expertise.The Victim Assistance project intends toto tackle this issue by providing technical trainings to the rehabilitation centers’ staff, thus developing their skills for the benefit of PwDs.Brief overview of the Iraqi contextAfter the official end of war in 2003, the population of Iraq is still hit by a complex and potentially violent situation. Despite a general improvement of the environment since 2007, the political agenda for the coming period is relatively volatile. Tensions between communities, issues such as access to oil reserves are not yet solved and can create further violence. As civilians are the primary victims of security incidents, Iraqi people continue to live in a climate of fear and insecurity. In addition, decades of wars and economic sanctions have also left Iraq in chaos. The country’s infrastructure has been severely damaged; access to basic goods and services remains restricted in many parts of the country. In Kurdistan Regional Governorate, where HI mission is based, overall situation and living conditions are far better than in the rest of the country. Nevertheless, the environment still remains unpredictable.

Overview of the 3 centers to be targeted by the assignmentKurdistan Organization for the Rehabilitation of the Disabled (KORD), Sulaymaniyah: the team is composed of 1 Physiotherapist (PT) and 3 Orthopedic Technicians (OT). The main users of service are adult amputees.Teaching Children Rehabilitation Centre (TCRC), Sulaymaniyah: this center focuses on physiotherapy for children with cerebral palsy, clubfoot and congenital hip dislocations.The team consists of1orthopedic surgeon, pediatricians, 16 PTs, 1psychotherapistand 5 OTs. Centre for Early Detection of Childhood Disability (CEDCD), Duhok:this centerfocuses on physiotherapy for children, mostly with cerebral palsy. The team consists of doctors, 7 PTs and 7 OTs.

JOB DESCRIPTION:

Goal, Objective and outcomes of the assignment:Goal: To improve the professional skills of Physiotherapistsin the chosen topics (see below) while promoting Person-Centered Approach (PCA) in RehabilitationSpecific objectives? To elaborate and carry out theoretical and practical training onthe chosen topics in each of 3centers? To contribute to promote Person-Centered Approach (PCA) and a multi-disciplinary approach in the centers? ? Cerebral Palsy Training ModuleTraining materials shall be designed and developed so as to update the knowledge of PTs about Cerebral Palsy (CP) and provide them with the newest insights regarding management of this impairment. Planning practice sessions is absolutely critical to ensure the retention of new acquired knowledge/skills and facilitate theirfurther implementation. The trainer shall ensure that at the end of the training, the participants will be able to:1. List the five main causes of cerebral palsy, as well as four excess and four deficit symptoms of cerebral palsy. 3. Describe accessory impairments common to children with cerebral palsy. 4. Categorize children with cerebral palsy into one of the three main types (according to the new classification of cerebral palsy) in order to define treatment, goals and approaches. 5. Describe through the ICF model the most important assessment tools for children with cerebral palsy. 6. Use the rehabilitation problem solving (RPS) form to get an overview of all the aspects of treatment of children with cerebral palsy, link the problems identified with the needs expressed by the families and identify the most appropriate treatment goals. 7. Describe the most important aspects of treatment/management of children with CPwith the newest evidence-based insights8. Describe the advantages of a multidisciplinary approach in the management of children with cerebral Palsy.

? Gait Analysis in children with Cerebral Palsy Training materials shall be developed so as to update the PTs onprinciples of the Gait Analysis used in childrenwith CP andcommon intervention modalities. The trainer shall ensure that at the end of the training, the participants will be able to:1. Describe the five priorities in normal gait according to Gage. 2. Describe the most common foot deformities in Cerebral Palsy and its effect on child gait. 3. Classify the gait of a child with cerebral palsy into one of the five main types of deviations and describe what kind of orthotics would be appropriate to improve the gait. 4. Use the 10 meter walking test, the 6 minutes walking test and the FAC to quantify the gait. 5. Use the Berg Balance Scale (BBS) to describe the balance of a child with CP and how to use it to analyze gait problems6. To build up intervention plan according to assessments findings with client/family involvement.7. To prescribe appropriate assistive devices according to the client capabilities and functional needs.8. Evaluate the evidence base for gait analysis in cerebral palsy.

? Assistive Devices (seating and positioning) training moduleThis training module willfocus on the seating and positioning techniques for children with CP and other neurological deformities. The trainer shall ensure that at the end of the training, the participants will be able to:1. Identify, prescribe and modify assistive devices (mobility aids and aids for activities daily living (ADL) for PwDs.2. Prescribe the appropriate assistive device (mobility aid and/or ADL aid) taking into account the demands of the user.3. Apply appropriate seating and positioning techniques for a person with disability taking into consideration:a. The specific consequences of neurologic impairments (abnormal movement and posture pattern / positions as a consequence of brain damage, spinal cord injury, spina bifida, muscular diseases, etc.) and / or orthopedic impairments (severe scoliosis, contractures, etc.)b. Find a balance between the functional and medical requirements.c. Make basic adaptations to the wheelchair to improve seating and positioning; straps, wedge, cushions, etc.4. To provide training on use of the specific assistive device to the user and/or his/her family.

? Documentation training moduleIn order to efficiently address the core concepts and skills inherent to the different documentation methods applicable to rehabilitation the training materials should be developed including concrete examples and case studies, so as to clarify the application of every relevant concept, skill and tool. At the end of the training, the participants should be able to:

  1. Describe the typical challenges of documentation practices and documentation methods.
  2. Describe best practices for documentation in rehabilitation, including evidence-based practice andPCA.
  3. Access tools and resources needed to document effectively.
  4. Understand the differences between the most common types of rehabilitation reports:a. Evaluation: Evaluation or Screening Reportand Reevaluation Reportb. Intervention: Intervention plan and progress report.c. Outcomes: Discharge/discontinuation report, and follow up.
  5. Use rehabilitation reports to improve the implementation of PCA so as to:a. Reflect on clinical reasoning and professional judgment.b. Communicate information about the client amongst the team within a multidisciplinary approachc. Create a chronological record of client status, rehabilitation services provided to the client, and client’s outcomes.The trainer shall also follow the following specific objective: providing physiotherapists with clinical training on different documentation methods applicable to the field of rehabilitation so as to facilitate the current systems’ transformation. KORD used to operate as an NGO focusing on the production of standard devices such as Below Knee and Above Knee prosthesis using resin system. On rare occasions the center would produce foot orthosis using polypolyne technology. The main users of service are adult amputees.Teaching Children Rehabilitation Centre (TCRC), Sulaymaniyah: this center focuses on physiotherapy for children with cerebral palsy, clubfoot and congenital hip dislocations, provision of technical aids such as seating and standing aids and delivery of orthotic devices. The team consists of1 orthopedic surgeon, pediatricians, 16 PTs, 1psychotherapistand 5 OTs. Some of the orthopedic technicians are PTs who have undergone on the job training to become technicians.Centre for Early Detection of Childhood Disability (CEDCD), Duhok: this center focuses on physiotherapy for children, mostly children with cerebral palsy. The team consists of doctors, 7 PTs and 7 OTs. The Orthopedic Unit provides devices, mainly Ankle Foot Orthosis (AFOs) and hip abduction braces as prescribed by doctors.

Through its Emergency Response Division (ERD), created in 2006 after the merge with Atlas Logistique, the organization is committed to:? Provide an adequate response to major natural disaster and conflict situations, anywhere in the world as much as possible within 72 hours; ? Provide assistance to vulnerable groups (refugees, IDPs, persons with injuries, older persons, children, etc.) affected by a crisis and maximize the number of beneficiaries. Within vulnerable groups, target especially PwD and meet their specific needs; ? Constantly monitor chronic crises and/or conflicts to ensure preparedness for and rapid response to eventual emerging needs.

MISSION CONTEXT

A critical humanitarian situationAccording to UNHCR latest figures from January 2013, there is between 2.5 and 3 millions IDP within Syria. More than 500,000 refugees are now registered in Syria neighboring countries. The current humanitarian response is far from answering to most urgent needs, growing by the day, with a partial coverage only.Moreover, amongst those victims of the conflict, an estimation of 8% (figures average from the UNHCR for refugees in neighboring countries, excluding older persons – if inclusion of Syrian IDPs and elderly, estimation would likely rise to between 15% and 20%) of them would be in an alarming situation of vulnerability, in terms of protection, access to basic services and in terms of coverage of their specific needs. Persons with mental or physical disabilities and/or chronic diseases, older persons, children head of households are in a critical situation, experiencing difficulties to access to already scare humanitarian resources.The situation of persons with injuries is also at stake. For those of them having benefited from emergency health care in Syria or host countries, facing the unavailability or lack of access to post-surgery services is likely to have a devastating impact on the prevention of further impairments, or to adapt to a new and challenging environment.

Summary of Mission Handicap International: Syrian CrisisAfter a first round of assessment on March 2012, HI emergency response division is implementing since May 15th an emergency intervention for the most vulnerable people affected by the Syrian crisis in Jordan and Lebanon, funded by ECHO, Centre de Crise (French MoFA), Swiss Cooperation and private donors, and aiming to support persons with injuries and other vulnerable groups (including persons with disabilities). HI is until now one of the only organization to address those issues through community outreach, and benefits from the trust of both communities’ representatives, local stakeholders and other field implementing organizations. In August 2012, HI initiated a basic needs assessment in Bekaa, leading to strong recommendations for winterization activities targeting vulnerable populations. Early September 2012, HI launched a new round of assessment in Turkey. After several requests from other relief organizations, a project is implemented in Hatay Province and in Northern Syria. This project focuses on physical rehabilitation care and psychosocial support.Simultaneously, after a technical assessment in Jordan and a rapprochement with ERW (Explosive Remnants of War) management authorities, the National Committee for Demining and Rehabilitation (NCDR), HI decided to launch a first risk education project addressing both Syrian refugees and local host populations living at the Syrian border.

Activities in Jordan

The situation of the most vulnerable persons (including persons with injuries) leads to implement a four dimensions intervention, in continuation of the previous phase (on-going project):

  1. Provision of support to hospitals in order to improve the provision of emergency rehabilitation care for 1,250 Syrian refugees with injuriesObjective: Provision of equipment, consumables, on-the-job training and accompaniment for local professionals of governmental health structures (Ramtha, Irbid and Mafraq) and to Dulayl JHAS's hospital in order to improve the provision of emergency rehabilitation care for Syrian refugees with injuries and contribute to better respond to needs related to the ongoing crisis

  2. Identification, assessment and specific support to ensure tailored response to the needs of most vulnerable Syrian refugees, including refugees with injuries and other vulnerable groups (through direct and indirect servicesObjective: Enhance the coverage of the needs (both basic and specific) of the most vulnerable persons through external referrals and personalized support to access services and/or internal referrals (to HI services)

  3. 1,250 conflict affected vulnerable persons with their family members (i.e. 6,250 beneficiaries) have a better coverage of their basic and specific needs through direct provision of services and/or referral upon needsObjective: Provision of direct emergency rehabilitation service care, complementary dedicated NFI, mobility aids and psychosocial support at community level (Mafraq, Ramtha, Irbid and Zarqa) and camps (Za’atari camp)

  4. Support to inclusion and accessibility for all in Za’atari camp through formal training to stakeholders and direct accessibility workObjective: Sensitization of stakeholders through formal trainings and dissemination of sensitization materiel in the clusters and accessibility in the construction of facilities in the camp (WASH, health facilities, school...).

MAIN RESPONSIBILITIES OF THE EXPATRIATEBased in Amman, under the direct responsibility of the Technical advisor, and with the support of the Specific Needs Technical Advisor at HQ level, you will be responsible for:? Provision of formal and on the job training of the physiotherapist team in the different area of intervention (Mafraq, Irbid, Amman, Zarqa’s governorates).? Support in designing, elaborating technical tools and trainings to support teams and stakeholders on physical rehabilitation related aspects and ensuring prior validation by the HQ TA of all technical tools (training tools, evaluation forms, leaflets, etc) used in the frame of your activities
